We recognize that every student's experiences are different, and review the nature of self-disclosed prior conduct before advancing an application for admissions consideration. Disclosing your prior conduct helps us understand the circumstances of your disciplinary history and evaluate your application fairly.

Why do I need to disclose my prior conduct?

IU Indianapolis requires current high school students to disclose any conduct related to academic or non-academic disciplinary actions taken against you at your school. The review committee will examine the situation of your prior conduct, the harm that occurred against others, and the positive changes you have made following the incident.

You must submit the online form below that provides factual details about the conduct you are disclosing.

Who will review my report?

100% of students with prior conduct are reviewed and individually considered by IU Indianapolis faculty and staff.

What should I include in my report?

As part of your application, you must provide a written statement regarding the conduct you are disclosing in the form below.

Provide a clear, detailed description of the incident, including a complete description of the facts and the circumstances that led to the charges. You should include:

  • Who participated
  • Who the victim was
  • What losses were suffered
  • When, where, and how the event(s) occured
  • The reason(s) for your behavior
  • What positive changes you have made in your life since the event(s)

This statement must also include a grant of permission to IU Indianapolis for complete access to criminal records, if any, and/or permission for complete access to disciplinary records, if any, at your previous high school, postsecondary institution, college or university.
