Instructions for filling out the form
Note: If your legal parents are not married to each other, answer the questions based on the parent who will provide more than 50 percent of your support from July 1, 2024 through June 30, 2025.
List all people that should be included in your parent’s family size, starting with yourself. You should include your parent, his or her spouse (if married), and any children or dependents living in the home if your parent will provide more than half of their support from July 1, 2024 through June 30, 2025.
If your parent pays child support for a child, do not include that child in your family size.
Sign the worksheet. Electronic signatures are not accepted. Make sure to provide your university ID on the worksheet to ensure there are no processing delays. Please note that in some cases additional documentation may be requested.
You can either submit online via email or secure upload or drop off a printed copy in person. Choose one of the following to complete your verification:
- Email the signed form to finaid@iu.edu, or
- Securely send your scanned documents using the upload tool. You’ll need to log in using your network ID and passphrase.
- Return it to the office:
