Provide your family size information through either of the following options. You do NOT need to complete both.

  • Log in using your IU account
  • Choose your campus
  • Answer questions to complete your worksheet
  • Sign with your mouse at the end.

Instructions for filling out the form

List all people that should be included in your family size, starting with yourself and your spouse (if applicable). You may list any children or dependents living in your home if you will provide more than half of their support from July 1, 2024, through June 30, 2025.

Sign the worksheet. Electronic signatures are not accepted. Make sure to provide your university ID on the worksheet to ensure there are no processing delays. Please note that in some cases additional documentation may be requested.

You can either submit online via email or secure upload or drop off a printed copy in person. Choose one of the following to complete your verification:

  • Email the signed form to finaid@iu.edu, or
  • Securely send your scanned documents using the upload tool. You’ll need to log in using your network ID and passphrase.

What’s next?

Please allow up to 15 business days for the Office of Financial Aid and Scholarships to review your documentation. Additional documentation may be requested, and any form that is incomplete or provides discrepant information will require follow-up and delay processing.

If we are unable to complete verification, we may be required to cancel any existing financial aid awards you might have received.
