  Upload any document securely. Make sure your name and university ID are included on anything you submit.

• Primary name change form

  Update your name if it has been legally changed or is inaccurate in our system. (You will need to provide supporting documentation.)

• Add/update SSN form

  Add or update your Social Security Number if it is missing or incorrect in our system. (You will need to provide supporting documentation.)

• Duplicate diploma and shipping request form

  Order a copy of your diploma. (If you want it to reflect a name change, submit the primary name change form above first.)

  Apply for resident student status (for fee-paying purposes) in Indiana.
